Le Kiss Note est un puissant carnet qui fait éprouver des sentiments réciproques à deux personnes dont le nom y est écrit. Cependant, ce dernier comporte certains risques, par exemple, si les deux personnes dont le nom est inscrit ne s’embrassent pas, la personne qui possède le cahier meurt dans les 24 heures qui suivent.
Ce carnet est possédé par un cupide nommé Guri. Par erreur, cette dernière écrit accidentellement le nom d’un lycéen nommé Seiji Aino. Pour éviter qu’elle ne meurt, elle lui demande d’embrasser une personne pour qui il éprouve une attirance. Son choix se porte sur la studieuse élève nommée Akane Hiyama, qui se révèle être une psychopathe folle amoureuse de Aino… Pour sauver Guri, va-t-il réussir à embrasser Akane ?
